DIRECTIONS FOR HOW TO USE THE SURVEY TO GIVE FEEDBACK TO TEACHERS
Dear Students:
Your opinions are important to us, especially when you share them in a responsible way. Your teachers would like some feedback about how they are doing. In past years we gave you forms to fill out on paper. Now you are going to give your opinions online about each of your teachers. We ask you to do it in a responsible manner, giving honest opinions in a respectful way. In this way, you will be helping your teachers improve, just as we want you to improve.
Here are the directions:
1/ Go to our website: www.riverschool.org
2/ Click on the page “For Students”
3/ Click on “Student Survey”
4/ Click on “Introduction Letter (PDF), Linda Inlay” and read the letter so that you know why we are doing this. Press the Browser's BACK button
5/ Click on “Teacher Evaluation Survey”
6/ Select one teacher at a time by scrolling down the “select “ box.
7/ Type in your “userid” in the “Your Name” box.
8/ Fill in the appropriate bubble for each question Q1 through Q29. Look at the top of the survey for what each number stands ford: 1=very rarely going up to 5= almost always. If it doesn’t apply, leave it blank.
9/ For Q30 and Q31: type in your answers. Please take this seriously because then your opinion will really count. Your answer about what you appreciate about this teacher helps because it lets your teacher know what to continue doing. And it makes them feel good for the all the hard work they do.
Your answer about you think would help them improve is also important.
10/ Then hit the “submit” button.
11/ Hit “Clear Changes” so that you can start on another teacher.
12/ Please do all your teachers: academic teachers, PE teachers, elective teachers.
